INDIVIDUAL COACHING AGREEMENT

We look forward to beginning coaching with you. Leadership Coaching has the potential to help individuals produce extraordinary results in their lives, careers and/or businesses. Coaching can prompt people to see things from a new perspective, generate new options, and bring clarity or focus. Working toward these results, however, will require effort on your part and may at times be challenging. The coaching process will require your very active involvement, commitment, and openness in order to produce the best, long-lasting effects because coaching is based on drawing out the perspective of clients, rather than on the coach offering overt advice. 

To optimize our work, all parties should feel comfortable sharing expectations and honest communication throughout the process. As we begin our work together, we would like to jointly agree to the following terms. Please indicate your agreement to the terms below by submitting your signature. 

As a client of In Three Generations…

1. I understand that coaching is a comprehensive process that may involve all areas of my life including work, finances, health, relationships, education and/or recreation. I acknowledge that any decisions about how to handle these areas, incorporate coaching into those areas, and implement my choices are exclusively my responsibility. 

2. I acknowledge that my coaching relationship with In Three Generations includes one-hour, virtual, individual coaching sessions with my assigned coach. If I do not carry out these coaching sessions within two years from the date of this agreement, I forfeit any remaining sessions and am not entitled to reimbursement for any unused coaching. 

3. I am aware that In Three Generations’ Coaches do not provide insurance, accounting, legal, cash management, investment, estate planning or other licensed services.  Any statement viewed as a recommendation by a Coach having accounting, tax, financial, or legal implications are only suggestions to be reviewed by my advisors, and any decisions I make throughout the coaching process are mine alone. 

4. I understand that canceled appointments delay our work and being considerate of one another’s time is a sign of mutual respect. If I need to reschedule or cancel an appointment, I will offer 24 hours notice. If I do not honor this commitment and miss a scheduled appointment, I will forfeit the session. 

5. Although the coaches and consultants within In Three Generations may be trained as psychologists or therapists, the work carried out in this coaching relationship does not include the rendering of any psychological services. Given that coaching is not a substitute for counseling, psychotherapy, or substance abuse treatment, I will not attempt to utilize it as such. 

6. I agree that if I am currently in therapy or otherwise under the care of a mental health professional, I have consulted with that care provider regarding the advisability of working with a coach and that this person is aware of my decision to proceed with the coaching relationship. 

7. I understand that if I would like for my coach to consult with other professionals, I will be asked to sign a release of information form, stating exactly what information is to be shared, with whom, and why.

8. Within the coaching relationship, as in any relationship, challenges in communication and connection may arise from time to time. If I am not satisfied with any area of our work, I will raise my concerns immediately and directly with my coach, who will maintain an open perspective and work together with me to resolve the identified concern. 

9. I understand that I may elect to discontinue coaching at any time. However, if I do discontinue coaching, I will notify my coach via email. Additionally, I understand that I am not entitled to having funds rendered for unused coaching services refunded.

11. Within the coaching relationship, as in any relationship, challenges in communication and connection may arise from time to time. If I am not satisfied with any area of our work, I will raise my concerns immediately and directly with my coach, who will maintain an open perspective and work together with me to resolve the identified concern. 

12. In the course of our coaching relationship, I may choose to disclose nonpublic information to my coach and understand that my coach will hold such information in confidence, not releasing it to anyone outside of In Three Generations, except as required under applicable law or governing codes of rules of professional conduct. If In Three Generations is served with a legal process mandating the disclosure of confidential information, I will be notified immediately.
